Transition
Secondary transition services are defined as a coordinated set of activities for a student with a disability designed with an outcome-oriented process that promotes movement from school to post-school activities. These activities are based on the individual student's needs, taking into account the student's preferences and interests. The activities include instruction, community experiences, development of employment and other post-school adult-living objectives and, if appropriate, acquisition of daily living skills and a functional vocational evaluation.
Post-High School Training Options
Many high school, college, university, community college, and technical school programs offer transitions/disability specialists who can assist with determining strategies for successful transitions. The best way to discover if a school has transitions/disability specialists is to contact the school directly.
